首先得澄清一下，此医保非彼医保，城乡居民医保和我们大多数人加入的城镇职工医保是两回事。而之所以要提高个人缴费，是由于这一新的制度由城镇居民医保和农村新农村合作医疗两大制度组成，如今要整合起来了，这种整合不仅能扩大基金抗风险能力，也能避免农民工同时在农村和城市参加两份保险的低效率重复；并能顺应全国户籍制度改革趋势，避免城镇居保和新农合重复建设。在新的制度框架上实现六个统一中，最首要的就是统一筹资政策，就是把原来较低的新农合筹资标准提高，从而也就需要提高新农合的个人缴费。因此城乡居民医疗保险要提高个人缴费的正确理解是参加新农合者的个人缴费有待提高，而和城镇职工保险参保者没有任何关系，不仅如此，根据中央十三五规划，大部分人参加的职工保险费率还有下降空间。
那新的居民医保个人缴费提高是否合理和应该呢？任何医疗保险，说到底是种互助制度，不管是城镇居民医保还是新农合，都是在居民或农民中互助，把钱集中起来，给生病者用。因此医保制度与过去的公费医疗和劳保医疗最大区别，就是需要自己拿出真金白银，或事前出或看病后拿，更多的是两者兼而有之。只不过居民医保和新农合所针对的人群分别是城镇没有工作的人和农村人口，这些人由于收入低于城镇职工，在缴费上面临困难，制度设计之初为了推动制度建立和扩面，各级政府财政给予了大量补助，财政出大头，个人出小头，使得我国从医保低覆盖国家急速成为广覆盖国家。
无论何种医疗保险，本质上和总体上是作为一种保险制度而非福利制度来安排的，需要有效率目标，医保制度的效率既来自于对医疗服务提供者——医院和医生的管理，又来自于对需求方的责任风险意识安排。这次是在提高政府补助标准的同时，适当提高个人缴费比重，在强调政府责任的同时增强个人的责任意识，更好地使用好医保基金，提高效率；与此同时，当医保总资金由于使用者日益增加、报销比例不断提高面临入不敷出时，受益于大量财政补贴而建立扩展的居民保险，如今受制于财政压力和预算紧缩，医保基金筹资来源自然只能从个人缴费增加入手了。
但我们同时要看到，医保制度又是我国社会保障制度体系的.重要组成部分，承担着收入再分配、实现社会公平的功能，这需要体现在缴费设计上，尤其是居民保险这一无工作人群的医保缴费设计。在这次城乡居民医保制度政策中，虽然已经根据人群情况酌情设计不同缴费，并通过不同档次让人选择；也提到逐步建立个人缴费标准与城乡居民人均可支配收入相衔接的机制，但政策没明确两者如何合理挂钩衔接来增进再分配。同时政策只模糊地提到加强基本医保、大病保险、医疗救助等衔接，鼓励集体、单位或其他社会经济组织给予扶持或资助，但对于缴费困难者如何补贴没有进一步说法。因此，居民保险制度建立虽然有增加个人缴费的合理性，但对居民保险人群还应进一步细分，不仅是让高收入者多缴，让穷困者病后有救助，更要帮助困难者一直缴得起医保金。
提高个人缴费引发关注，相当一部分原因是媒体选择性突出制度系统改革中“个人缴费增加”这一小点，而忽视制度整合的全局性系统性安排，给公众造成了居保制度改革的要点就是增加缴费的片面印象。当然，对居民医保参加者来说，一般也搞不清自己是保险还是福利，一直把财政补贴当“福利”在享受，如今要以个人缴费来弥补基金不足，必然有“福利”下降的感受。
对于社会医疗保险是什么，长期以来我们在认识上还是比较模糊的。现在是时候明确，医保制度是取之于民、用之于民，同时又是社会互助共济的重要方式与手段。这一点我们一定要讲清楚、弄明白，也要通过各种方式让群众理解认知，否则大家都是稀里糊涂，茫茫不知所然，再加上某些媒体专业素养匮乏，必然造成对造福民生的好制度产生相反的误读与质疑。
